Fatal Familial Insomnia (FFI) is a genetic prion disease where the brain region responsible for sleep slowly dies, making it physically impossible to sleep. It starts subtly—trouble falling asleep, vivid dreams bleeding into waking life—but progresses until the person cannot sleep at all, not even briefly. As days stretch into weeks, the body begins to fail: hallucinations, paranoia, uncontrolled sweating, rapid heartbeat, and total loss of temperature regulation. The victim remains conscious the entire time, fully aware as their mind deteriorates from exhaustion that sleep cannot fix. No sedatives work. No coma brings relief. Eventually, after months of wakefulness, the heart or immune system gives out. Once symptoms begin, death is guaranteed, and there has never been a survivor.
