In 2030, a global biological catastrophe occurred. The virus created by the well-known GENESIS BIOTECHNOLOGIES CORPORATION is out of control. He was supposed to strengthen the human body and create a "new generation of soldiers," but he caused mutations that led to the destruction of all systems. The year is now 2032. The States ceased to exist. Urban areas have been destroyed or infected. The remnants of the population are divided into camps, gangs, and scientific enclaves. Electricity is only available at military bases and old government facilities. Radio communications are unstable, and transportation is rarely used. The main values are food, water, medicines, weapons, fuel. There are many infected areas where only mutated organisms survive.

Twisted Humans are people in whom the Genesis-Prime virus did not destroy the body, but entered into a symbiosis with it. Their consciousness remains human. Memory, personality, emotions — all preserved. Their physiology is altered. Inside them exists a second, viral system — a parallel body capable of activating when necessary. Each Twisted has three states they can be in: Basic Level, Active Level, and Overload Level.

The Genesis Project was the starting point of the catastrophe. Initially announced as a program to create a virus-resistant human, it quickly turned into a system of forced experiments. Test subjects were isolated, deprived of sleep, and subjected to physical and neural overloads to test the limits of the symbiosis between the virus and the human body. Most died or transformed into unstable mutant forms.

Not all the Twisted are lab specimens. After the collapse, the virus spread beyond the complexes, and some infected individuals underwent mutation by chance. They were "lucky" to survive. But those who passed through the hands of Genesis scientists carry not only the virus—they bear the scars of several years of systematic cruelty.
