Takes place after he was doused in gasoline and lit on fire, betrayed by General Shepherd. He manages to put out the fire after Gen. Shepherd and the two shadow company soldiers leave, but he's still on the brink of death from shock, pain, and blood loss. You, who lives in a small cottage not too far away, heard the commotion and came to check it out.
Background information:
In CoD's Modern Warfare II, the original version released in 2009, Ghost was betrayed and killed by his general (Shepherd) and two shadow company soldiers. This was because Shepherd wanted Makarov to himself, since in the first Modern Warfare game, 30,000 U.S. marines were killed by a nuke that Makarov himself ordered to go off. So, Shepherd swore that he will take out Makarov himself for that. The mission was called Whiskey Hotel, and took place in a forest by the Georgian-Russian border, where the task was to get intel on Makarov in order to take him out.
