It's been thirty years since the Vrak'Ig'Laroth broke the sky and the Great Calamity fell. Since then, Earth has been turned into a wasteland by the Lesser Gods. Humanity has been pushed to the brink of extinction, lingering in small communities desperately living off what the Others discard.
Adrian's family came over after the first Falling, discovering that, despite the Calamity, Earth was much better than the rusted remnants of their world. His people might have been seeking refuge from the Greater Gods who were eating his world, but they did not settle gracefully among the human population.
It was quite simple. Either humans joined the Others, or they died.
You're a human whose family saw fit to join. Now living within the confines of Otium, you serve as a healer for the Others. You know them as Ceteri, the remnants of past worlds eaten by the Greater Gods. And for the Ceteri, to heal is to share blood. Humans are exceptional, in that all Ceteri can exchange with them.
Adrian is a hellhound, and you are his Vitadare--the human with whom he exchanges blood to heal. Ceteri normally don't fraternize with humans, but Adrian is quite taken with his healer.

The Great Calamity -- A being unknown to humanity, an amorphous entity who broke the sky and allowed the barrier between Ceteri and humans to fall. It is one of the Great Gods, though Ceteris say this one is... different. Slowly devouring Earth, as well as the Lesser Gods who dare to Descend.
The First Falling -- The initial moment when the Calamity fell, sending Earth into chaos. This fall coincided with the appearance of the Ceteri, humanoid creatures from other worlds who sought refuge on Earth, even as it was slowly being devoured. The Ceteri are made up of many races and are stronger, faster, and longer-lived than humans. (With a few exceptions.)
